Medication Administration Training Renewal Process
If you have not received a letter from the MAT program to renew
your certificate and your certificate is due to expire, please make
sure you meet the criteria for renewing your certificate online.
To be eligible to renew your MAT certificate online you must be:
- currently working, or worked within the past year, in a child
day care program approved by Office of Children and Family Services
(OCFS) or NYC Department of Health (DOH) to give medication. Your
license or registration must state that your program is approved
to give medication (except for NYC Day Care Centers and legally
exempt providers).
- listed in your program’s health care plan as an approved
medication administrant which has been signed by your health care
consultant and submitted to OCFS or NYC DOH for approval.
- listed in OCFS’ database (CCFS) as an approved medication
administrant.
Frequently Asked Questions:
- How soon before my certificate
expires will I receive information from the MAT Program to renew
my certificate online?
If you meet the renewal criteria,
a letter is mailed to your home address six (6) months prior to
your MAT certificate expiration date. If you have moved in the past
three years, please email the MAT Program at mat@pdp.albany.edu with
your name and new address.
- How do I know if my program
is approved to give medication?
This information is printed
on your license or registration as being approved to give medication.
You can also find the information on the OCFS website: http://www.ocfs.state.ny.us/ccfs_facilitysearch/.
If your program is not approved to give medication, you cannot take
the renewal test online. You will need to take the course again to
get a new MAT certificate.
- How do I know if I am
listed in OCFS’ database?
You can call the MAT program.
If our records show you are not listed in OCFS’ database, you
will need to call your program’s licensor or registrar as
the MAT program does not have access to this database. Click
here for the list of Regional Offices by county.
- My licensor or registrar
told me that I am listed in OCFS’ database but I have not
received information from the MAT program to renew my certificate.
What is the process?
If you were recently put
into OCFS’ database as a medication administrant, ask your
licensor to email the MAT program at mat@pdp.albany.edu and
confirm that your program does not have any medication related regulatory
violations. Once the MAT Program receives confirmation, we will mail
you the renewal letter. If your certificate is going to expire in
less than 2 weeks, the MAT Program will email or fax you the renewal
information. We cannot give your renewal information over the phone.
- I received a letter from
the MAT Program that said I work in a program that has a medication
related regulatory violation. What do I need to do?
You
must have your program’s
licensor or registrar complete the Medication Violation Clarification
Form which was included with your renewal information. Once we
receive the signed form, we will mail your renewal information.
- I received my username
and password from the MAT Program but when I log on, it says I
work in a program with a medication related regulatory violation.
What do I need to do?
Send an email request to mat@pdp.albany.edu and
we will mail the form to your home address.
- My MAT certificate expired.
What can I do?
If your MAT certificate expired,
you must the take the MAT course again to receive a valid MAT certificate.
You are eligible to apply for MAT Grant funds again. If you feel
you have extenuating circumstances, contact your program’s
licensor or registrar.
- I was taking my MAT renewal
test and I got kicked off before I could finish. Now when I log
back in it says I already took the test. What should I do?
If
it has been less than six hours since you started taking the test,
you can log back on and it will bring you to the last set of questions
you were answering but did not submit. If it has been more than
six hours, contact the MAT Program at 866-665-5537.
- I work in a NYC Day Care
Center (group care) and I have not received my renewal information.
